NO APPOINTMENTS ARE SCHEDULED ON SUNDAYS.
Please leave a message that includes your name, telephone number, e-mail address, length of session you are requesting and two or three possible times you are available. Your call/message will be returned promptly to schedule a mutually convenient time.
The exact business address will be given to you at the time we schedule your appointment, as I DO NOT accept walk in clients.
New clients will be asked to fill out a confidential health history/client intake form prior to the first session. As with all health care professionals, client confidentiality is always respected. All client/therapist relationships will remain confidential. Refusal to complete the intake/health history is cause for cancellation of the appointment.
CANCELLATIONS AND LATE ARRIVALS
Cancellations are accepted within 24 hours of the scheduled appointment without penalty. Emergencies happen and I understand that. However, if a client is habitually canceling or not showing for their scheduled appointment, the fee for the missed session must be paid before re-scheduling the next appointment.
Clients who are late for their appointments must realize that the same fee for the full appointment applies, and in most cases the appointment will end at the planned time regardless of the start time.
I will make every effort to inform clients in a timely manner of my need to change appointment times.
CANCELLATION POLICY
First time clients must provide a valid MasterCard or Visa account number, validation code, expiration date and mailing address to hold their scheduled time. If you do not arrive for your scheduled appointment, half the value of the scheduled service will be charged to your account.
